These individuals have a diagnosis of Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD) as well as high medicals costs often associated with chronic health conditions. This position is responsible for assisting identified individuals in establishing appropriate behavioral health and other necessary services which promote overall improved wellness and health outcomes.What you will do:
      • Reviews Disease Management Cohort List in a timely manner and prioritizes outreach attempts based on clinical factors including, but not limited to, medical necessity, ER or hospital utilization, Medicaid costs, or recent substance use conditions.
      • Utilizes motivational interviewing techniques and a person-centered approach to engage individuals identified through the cohort to tailor services to the needs and wishes of the individual.
      • Coordinates with intake department to ensure timely admission into clinically appropriate program to meet individual treatment and recovery goals.
      • Completes all required documentation for the Disease Management program, which includes but is not limited to: review and update cohort lists as a result of outreach and engagement efforts and ensures all state reporting is completed in CIMOR per program requirements.
      • Promptly responds to case management or crisis intervention needs of individuals on DM cohort until permanent case management assignment is made.
      • Assists clients in gaining appropriate access to resources, both internal and external which support their individual needs and overall well-being.
      • Collaborates with Healthcare Home (HCH) Department to make sure that all Disease Management individuals are enrolled in HCH in order to address healthcare conditions, promote and encourage wellness through education, as well as healthy lifestyles and preventative care.
      • Participates in all trainings, conference calls and community wide meetings associated with the Disease Management Program and effectively communicates any status updates or changes to Disease Management Team Leader.
      • Serves as a vital team member of a multi-disciplinary treatment team for the Disease Management Program and the Community Psychiatric Rehabilitation (CPR) Department.Qualifications:
        • Bachelor's degree in psychology, social work, nursing, counseling or closely related human services field. If the degree is in a field other than human services, a bachelor's degree and four years of experience in mental health is required.
        • Equivalent experience may be substituted on the basis of one (1) year of experience for each year of required educational training.
        • Must maintain a valid drivers license. Must have access to dependable auto with adequate auto insurance and registration, for making frequent home/community visits.Available to work weekends and evenings if required by department.
