Centerpoint Medical Center!Job Summary and QualificationsServes as
a patient/nurse/provider liaison in the care pathway for the heart
failure patient population: manages daily census list, oversees
documentation compliance to standards of care and core measures,
collaborates with clinical staff and providers to ensure evidence
based standards of care are being followed and communicates with
care team any follow up needs for patient course of care.
Participates as a team member for the Cardiovascular Services
Multidisciplinary team and committee. Responsible for performance
improvement projects related to clinical performance variances with
clinical teams and committees.Collaborates with Cardiovascular data
abstractors/analysts and the ACC/NCDR/AHA to ensure current
evidence based standards of care are in compliance. Communicates
any variances in care to providers as concurrent as possible.
Supports the quality improvement program and manages the
performance improvement projects for the Heart Failure program.
Manages the GOLD PLUS status for Get with the Guidelines with
American Heart Association for the Heart Failure program and
ensures that the standards of those designations are adhered to.
Collaborates with Heart Failure Medical Director to ensure
development and compliance with documented policies and procedures.
Collaborates the Nursing Education Department to coordinate Heart
Failure educational efforts directed to the hospital staff. -
Education Summary: Bachelors in nursing required
Licensure/Certifications: Must hold a current license from the
Missouri Board of Nursing. BLS/ACLS required. Certification and
organization membership is strongly recommended through a society
focused on ACS treatment and diagnosis. Experience: Three years of
nursing experience in the specialty area of cardiology with
demonstrated competence. is a 285+ bed hospital featuring
state-of-the-art equipment and technology. Our hospital offers some

Center. Our Orthopedic Services include Total Joints and Sports
Medicine. We are an accredited Cancer Program and Breast Center. We
provide Women's Services, a Level III Neonatal Intensive Care Unit,
and a Stroke Center. Our Outpatient Services include a Surgery
Center, Advanced Wound Care Center, Pain Center, Sleep Disorder
Center, Imaging, and Rehab. We are part of HCA Midwest Health, a
