Job Title: Team Supervisor/Health Coordinator Department:
Community Living Reports To: Program Manager Supervises: Direct
Support Professionals FLSA: Non-Exempt Pay Rate: Minimum $23 Hr.
SUMMARY A Team Supervisor provides oversight of day-to-day
functions as it relates to their work-site location, including
recruiting, training, and supervision of direct support
professionals, personal care assistance and teaching; community
integration and socialization; development of personal interests,
skills and talents; and health coordination and oversight. Works
with all members of the team in the home to ensure the highest
quality of medication administration, food purchasing,
implementation of menus to meet individual diets of the residents,
and to ensure the cleanliness of individuals and their environment
at all times. BASIC FUNCTIONS AND GENERAL RESPONSIBILITIES :
Responsible for ensuring work location is adequately staffed,
trained and compensated. Provide training to newly retained Direct
Support Professionals assigned to work-site location. Provide
timely feedback to Direct Support Professionals in the areas of
performance, processes and procedures, and schedules. Recommend
corrective action, up to and including, termination for Direct
Support Professionals assigned to work-site location. Maintain
staffing schedules for work-site location. Provides direct coverage
during times of staff shortage. Provides 24 hour on call Support to
Direct Support Professionals at their work site. Ensure employees
hours are recorded appropriately and pay period ending approval
process is completed according to pay schedule. Ensure the services
provided are of the highest quality and best practice. Ensure
activities offered and provided are person-centered. Provides
support on an individual basis in making informed choices, growing
in self-esteem and personal dignity, and achieving community
integration and participation. Ensures all Direct Support
Professionals understand and provide services in accordance with
each individual plan. Recommends goals to be incorporated into the
ISP. Ensures the spiritual and emotional needs, socialization
needs, and all personal care needs are provided according to best
practice. Available, as needed, to address emergency situations and
to assist in problem-solving with staff. Ensures documentation is
compliant will all applicable laws, rules and regulations. Ensure
electronic records and all health and safety records are complete
at the beginning and end of each staff schedule. Ensure event
reports are completed and submitted according to agency policy and
rules and regulations of all regulatory/governing bodies. Ensure
all Medication Administration Records, physician orders, and other
medical forms are current. Ensure tracking system for medical
appointments, follow-up appointments and routine lab work is
current and accurate, including Therap calendar. Ensure
confidential and health related records are kept secured. The
ability to effectively and efficiently use computers or mobile
devices to input, access, modify, or output information. This
includes the ability to quickly access, store and retrieve data.
Knowledgeable all health and safety guidelines, rules, policy and
procedures. Ensure the health and safety of all individuals through
teaching and providing support in areas of personal care needs,
dental hygiene and other health-related RN delegated duties.
Contribute to a home-like, safe, sanitary environment for
person-centered living, following up/reviewing items checked off on
chore logs/supply lists to ensure supplies are available and
environment is maintained to highest standards. Keep hazardous and
potentially harmful materials locked and out of individuals? reach
unless the individual has demonstrated safe handling skills. Ensure
soiled laundry/trash is treated/disposed of according to safety
guidelines. Know and observe guidelines for drills and emergencies.
Support individuals in arranging and participating in medical
appointments as needed. Attends medical appointments as necessary.
Provide daily and monthly medication administration oversight by
ensuring physician orders are current, receives monthly medication
packets from the pharmacy. Administer medications and monitor
individuals who self-medicate according to guidelines taught in
Medication Administration Certification classes. Ensure all
personal care items that are on the MAR or in the ISP are carried
out; shaving-face/legs/underarms; nail trimming, etc. Provide
professional and timely correspondence to physicians, guardians and
Manor staff informed of individuals health issues. Ensure
medication errors are reported and tracked according to rules and
regulations. Maintains agency and individual funds assigned to
work-site location. Reconcile all finance accounts and ensure funds
are accounted for according to agency policy. Ensure individuals
and Direct Support Professionals have the necessary funds to
participate in chosen activities. Ensure all funds are stored and
locked according to agency policy. Work with program manager and
finance department to ensure all NAFS funds are allocated
appropriately. All Other Duties as Assigned ORGANIZATIONAL
RESPONSIBILITIES : Protects the rights of all individuals: Is
knowledgeable about the rights of people with
intellectual/developmental disabilities. Respects and advocates for
the capabilities, dignity and rights of each individual and files
report any human rights violation immediately. All forms of abuse,
neglect, and exploitation are unacceptable. Responsible for
reporting any observation or suspicion of these to a member of
administration. Provides regular and timely communication/feedback.
Conducts one (1) group house meeting monthly with all individuals
living at their assigned location. Provide regular and consistent
communication with Program Managers and members of administration
to promote professional collaboration between departments. Provides
regular health updates to the RN and/or Program Manager and seeks
clarification when necessary. Physical Expectations This position
requires extended periods of walking and standing with some periods
of sitting, which is dependent up the needs and activities of the
individuals supported. It may require light to heavy lifting and
occasional stooping, kneeling, bending, and/or climbing stairs. The
position will require occasional physical intervention if
confronted by a person displaying combative or potentially
dangerous behavior. Driving an agency or personal vehicle is a part
of community integration for the individuals we support. Working
Conditions Office/home environment with noises from appliances,
telephones, and occasional small machinery (lawn mower, etc.). May
be exposed to bloodborne pathogens and/or infectious diseases.
Occasionally, prolonged hours or unusual chores to meet the
unexpected medical needs of the individuals. QUALIFICATIONS Must
have a high school diploma or General Equivalency Diploma (GED).
Must have at least one (1) year of successful employment in a work
environment similar to that described in this job description, OR A
combination of medical education and work experience may be
substituted upon the approval of the Director of Nursing and Chief
Executive Officer. Must obtain and maintain training and
certification in all required areas such as Abuse/Neglect, CPR,
First Aid, Level 1 Medication Administration, Positive Behavior
Supports, TOOLS and Crisis Intervention. Must have reliable
personal vehicle for transporting clients. Must obtain a Class E
driver?s license during new hire orientation? must carry minimum
coverage applicable by law for auto insurance Must Receive Seasonal
